The doctors at Chan Chiropractic regularly perform sports physicals for the Poway Unified School District high schools and local athletic associations. Dr. Casey Chan offers a significant sports physical discount to all youth athletes under the age of 18.
What do you need to get a sports physical at our clinic?
1.) Fill out the required forms. The Poway Unified School District has required specific forms that need to be filled out by the student, parent/legal guardian, and doctor. To be on time for your appointment, we highly recommend that the student and parent fill out the patient-portion of the forms prior to your appointment. The forms are often lengthy, and it is important to fill them out accurately. Other athletic activities not associated with the Poway Unified School District will most likely have their own forms as well. Bring in all the paperwork to your appointment.

2.) Children under 18 years old must be accompanied by a parent or legal guardian at the time of his/her appointment. Signed consent to proceed with the examination is required. If applicable, proof of legal guardianship will be required.
3.) Schedule an appointment with our office when you have an idea of when both the parent/legal guardian and student is available for an appointment. Due to the high volume of athletes we see, we no longer take walk-ins.
4.) Payment is required at time of service.
5.) If the child has significant illness or injury, we recommend that the student see his/her own doctor for participation clearance.
